Lymphlo Independent Contractor Agreement

Please review and agree to the terms below before working with Lymphlo. This agreement outlines expectations, compensation, and responsibilities as an independent contractor.

Therapist Information

Contractor Status

I understand that I am an independent contractor and not an employee of Lymphlo. I am responsible for my own taxes, insurance, and business expenses.

Scope of Work

I agree to:

• Perform scheduled client appointments

• Deliver services professionally and on time

• Represent Lymphlo in a positive manner

• Follow all Lymphlo procedures and guidelines

Compensation

• I will receive $75 per completed appointment

• Payments will be issued on a biweekly basis

• Only completed appointments are eligible for payment unless otherwise approved

Tips & Follow-Up Emails

• Lymphlo may send a follow-up email after each appointment

• This email may include an option for clients to leave a tip

• I understand I must submit all required appointment information for this email to be sent

• Tips are not guaranteed and are not part of base compensation

Scheduling & Attendance

• I will maintain accurate availability

• I will show up on time for all accepted appointments

Cancellations & No-Shows

• I will provide at least 12 hours notice for cancellations when possible

• Repeated cancellations or no-shows may result in termination of contract with Lymphlo.

Standards of Service

I agree to:

• Provide high-quality service

• Maintain professionalism with clients

• Follow all safety and sanitation standards

• Protect client confidentiality

Service Restrictions & Compliance (No Squeeze Therapy)

Lymphlo strictly prohibits the use of unauthorized techniques, including but not limited to “squeeze therapy.”

Contractors agree:

• Not to perform squeeze therapy under any circumstance

• Not to advertise or suggest squeeze therapy to Lymphlo clients

• To follow only approved Lymphlo service methods and techniques

Failure to comply with this policy may result in immediate termination of this agreement.

Non-Solicitation

I agree not to solicit Lymphlo clients outside of the platform during my time with Lymphlo and for 6 months after.

Equipment & Responsibility

I am responsible for providing and maintaining my own tools and materials unless otherwise specified.

Termination

This agreement may be terminated at any time by either party. Lymphlo may terminate immediately for misconduct or failure to meet expectations.

Liability & Confidentiality

I understand:

• I perform services at my own risk

• I am responsible for any required insurance

• I will not share or misuse client or business information
